The accident occurred around 7:24 p.m. near Johnston Drive when Phan, driving a 2016 Kia Forte northbound, crossed the centerline and collided with a southbound 2017 Ford Explorer. The impact caused both vehicles to veer off the road, with the SUV, driven by 37-year-old Jason Lyle from Lewis Center, coming to a stop on the right side of the road, along with Phan’s Kia.

Lyle, who suffered serious injuries, was rushed to Riverside Methodist Hospital. Two young passengers in the Ford Explorer were taken to Nationwide Children’s Hospital, where they were treated for minor injuries.
